The doctors welcome new patients who live within our practice area.
The registration form will ask you for personal details such as your name, address, date of birth, telephone number, previous address and doctor. This information helps the Primary Care Trust to quickly trace your medical records and send them to your new surgery. The new patient questionnaire will ask you for some basic, lifestyle and allergy questions, so we can provide the best service possible from the start. Please read, print and complete the required information within the New Patient Registration Pack and also print and complete the Registration Form GMS1 (clicks on links below). Alternatively call into the surgery to pick up the forms.
Once you have completed and signed the forms bring them along to the surgery after 2pm Monday to Friday to be registered with the practice. You will be required to produce proof of address and this can be a recent utility or council tax bill to enable us to complete your registration.

New Patient Medical
We offer all our new patients a medical check with our practice nurse or Health Care Assistant. This will consist of a short medical examination e.g height, weight, blood pressure, urine testing, medication review and the nurse takes the opportunity to ask about previous medical history.
