Last Updated: Thursday, 21 September 2023Our Pharmacist works as part of the practice team to resolve day-to-day medicine issues and consult with and treat patients directly.
The role of the clinical pharmacist includes:
- Offering medication reviews for patients with long-term conditions such as asthma and hypertension (high blood pressure),or patients on multiple medications.
- Seeing new patients who take a number of medicines to ensure that their current medication and appropriate medical follow-up is planned.
- Dealing with patient prescription queries and offering telephone consultations with patients who have a medication related query.
- Ensuring changes made to your medication following a hospital admission or out-patient visit are updated on your repeat prescription.
